The Process Failure Modes and Effects Analysis (PFMEA) diagram is a tool used to evaluate possible failures and how they may be caused. It details the various problems encountered during the assembly and the countermeasure actions required to fix the problems. The diagram consists of the process function, potential failure modes, potential failure effects, potential causes of failure, current process controls, recommended actions, responsible person and target date, and the taken actions. Through these steps, this diagram allows one to see the potential failures in the production process along with how much risk is associated with each. It also organizes these potential problems in order to do away with or decrease the probability of these errors occurring in the future.

The first step, the process function, is an action that must be completed in the assembly process. Examples of this would be connecting wires to springs, connecting the speakers, connecting the AM or FM tuners, etc. The second step lists the potential defects or errors that could occur. After the steps have been listed along with their potential failures, the third step of current process controls begins. At this step, you identify what the possible effects of a defect or error could be. For example, something like the batteries did not work then the radio wouldnt work at all. This problem would be more extreme because it makes the entire process useless, and this would be rated a 10. The next step is recommended actions. This process identifies the potential causes to the problems that are stated. These are specific errors that can be fixed or controlled. For example, not handling the wires carefully and breaking them. This is something that can be fixed and controlled, therefore is a potential cause of a specific failure mode.
